Barrel Cactus

I received a barrel cactus from my children 3 years ago. It thrived and grew quite a bit. I transplanted it about 2 months ago and noticed it has a brown spot on the back and has gone somewhat soft. I used cactus soil to transplant and was very careful. It has 2 little "arms" growing on the sides, can these be saved? Help I would love any comments on this...
